Gynecomastia may occur at any age in the lifetime of the patient. There are three main milestones during development where the chances of developing gynecomastia may be higher,
This condition can be seen in infants in the initial stage of life. This happens due to the circulation of the estrogen hormone that was transferred to the infant from his mother when in her womb. This condition is usually self-limiting and as the levels of estrogen decrease, the condition also regresses.
Some may develop gynecomastia during puberty, in this group of patients this condition may be self-limiting. If it is not self-limiting, this condition progresses over the period making the enlargement harder. Surgery for such kind of patients is performed after necessary investigations. The surgery includes performing liposuction to suck out the excess fat that has been deposited. In most cases, the removal of excess fat is good enough to correct this condition. Few patients many develop enlargement of the glandular tissue in the nipple-areola region which might be painful. Such kind of patients may require excision of the glandular tissue with a very small access incision over areola.
As an individual grows older, there are chances that he will put on some weight over the chest region and develop gynecomastia. This is due to the gradual decrease in the levels of testosterone. This condition requires liposuction of the deposited fat. If there is glandular growth also, then the excision of the glandular tissue is performed.
Apart from the three main groups that are prone to developing gynecomastia, there are few other conditions that may cause gynecomastia like bodybuilders who consume anabolic steroids and massive weight loss patients.
Due to less amount of fat content and more amount of glandular tissue, the surgery for bodybuilders who develop gynecomastia due to the use of anabolic steroids would be to surgically remove excess glandular tissue with a little amount of liposuction
Patients who lose a lot of weight over a short period of time have loose skin along with resistant fat and the gynecomastia resembles that of a female breast with significant ptosis. The extent of ptosis causes the nipple to be present at a much lower level than in normal males. Such patients usually require extensive surgery to remove the fat, remove the excess skin and reposition the nipple to an appropriate position.
Gynecomastia surgery is usually a daycare procedure preferably performed under General Anesthesia where the patient can go home the same day.
